1. What services do you provide?
We provide comprehensive home health care services designed to support clients safely and comfortably in their own homes.
Our services include:
• Skilled nursing care
• Medication management and administration
• Wound care
• Assistance with activities of daily living such as bathing, grooming, dressing, mobility, and toileting
• Care plan development and ongoing monitoring
• Vital sign monitoring and health assessments
• Chronic condition support and education
While we provide general behavioral health support within a home care setting, we do not provide specialized psychiatric or complex medical case management. When a higher level of psychiatric or acute medical intervention is needed, we assist families by referring them to appropriate local agencies and specialists.
2. What qualifications and licenses does your team hold?
Our clinical team includes Registered Nurses (RN), Licensed Practical Nurses (LPN), and Certified Nursing Assistants (CNA). All licensed nurses are properly credentialed in the State of Maryland and are insured. All clinical staff maintain current Basic Life Support (BLS) certification.
Do you conduct background checks?
Yes. We conduct comprehensive background checks for all employees and contractors prior to hire. In addition, client intake involves review through support planners and formal assessments. A Registered Nurse performs an initial in-home assessment before services begin.
3. How do you ensure patient safety?
Patient safety is maintained through a thorough RN initial assessment, evidence-based risk assessment tools, individualized care plan development, ongoing RN supervision, and routine documentation review.
How do you handle emergencies?
In a medical emergency, 911 is activated immediately. The client is assessed, and responsible parties are notified without delay.
How do you assess fall risk?
We use the Morse Fall Scale during admission and update it as needed.
How do you supervise staff?
We utilize a geo-fenced timekeeping system and frequent random RN supervisory visits.
Re-evaluation: Care plans are reviewed monthly or immediately updated if a condition changes.
4. How are care plans developed?
Initial Assessment: The RN completes a head-to-toe assessment (Morse Fall and Braden Scales, skin/body systems review, vital signs). Every plan is tailored to the client’s medical condition and family goals.
Coordination & Physician Role: Communication with primary providers is maintained prompts updates as medically necessary.
Family Participation: Client consent is required prior to sharing info. In Maryland, required forms include:
• Service Agreement & Consent for Treatment
• HIPAA Authorization for Release of Information
• Advance Directive & Emergency Contact Form
• Notice of Privacy Practices Acknowledgment
5. How do you handle medication management?
Administration: Medication administration is performed by a Registered Nurse in accordance with physician orders. All meds are documented in a Medication Administration Record (MAR), which is monitored for accuracy.
Error Prevention Protocols:
• Verification using two patient identifiers (name and DOB)
• Review of physician orders prior to administration
• Clear documentation in the MAR and ongoing RN oversight
6. How do you communicate with families?
Regular Updates: Yes. We provide regular updates via phone communication and as otherwise agreed upon in the care plan.
Point of Contact: Families may contact the assigned supervisor or Nurse Manager directly with any questions or concerns.
After-Hours: Support is available for urgent clinical concerns outside of standard business hours.
7. What makes your agency different?
Advanced Training: We conduct structured training at least once every three months, exceeding minimum skill validation requirements.
Specialized Referrals: We provide general home care and offer referrals for high-acuity psychiatric or complex medical cases needing specialized intervention.

8. What are the costs and payment options?
Costs vary by clinical needs. Maryland hourly rates range from:
• $25–$35 for non-medical personal care
• $40–$75 for skilled nursing services
Payment & Policies: We accept private pay (Check, CashApp, Wire, Cash), Medicaid, and certain insurance plans. Minimum service requirement is 40 hours. All care is provided under a formal service agreement.
