Our comprehensive initial assessment begins with a thorough head-to-toe evaluation by a registered nurse, including Morse Fall Scale and Braden Scale assessments, complete skin and body systems review, vital signs monitoring, and detailed medical condition documentation to establish a safe and effective care plan. Every plan is individually tailored to the client's specific medical needs, functional requirements, and family goals, while maintaining ongoing communication with primary care providers to ensure adherence to physician orders and coordinated care. All services require proper client consent and comprehensive documentation compliance with Maryland state regulations, including Service Agreements, HIPAA Authorization, and other required forms to ensure privacy, safety, and personalized care delivery.
